SOIC Employment Checklist

Congratulations on your research or teaching appointment. All newly hired employees are required to fill out the following three employment forms and bring them with you when you sign your contract.

Required Employment Forms

Complete the following three forms, print them out and bring them with you when you sign your contract.

  1. Personal Profile NOTE: In the spot that says "Home Address", you must use your Bloomington address.
  2. Background Check Form: You just need read through the information and then complete the form and sign page 1, "Consumer Disclosure."
  3. IU Employment Application Form

Contract Signing Information

When you sign your contract, you are required to bring with you the following items:

Domestic Students must bring one of the following items:

  • Valid Passport
  • Valid Driver's License
  • Photo ID

And ONE of the following items:

  • Social Security Card
  • Birth Certificate

International Students must bring the following items:

  • I-20
  • DS-2019
  • Passport with I-94 Form
  • Social Security Card (if you have one)

Note: If you are an international student, you may not have a social security number yet. When you sign your contract we will give you information on how to get a social security number. Your hiring paperwork must be completed before you apply for a social security number.

Getting Paid

The  University requires that all university employees (including staff, faculty, graduate students, and hourly) utilize direct deposit for payroll. Once your hiring paperwork has been completed and entered into the human resources system, you may then request direct deposit into your bank account.
